Office Policies
When calling our office to schedule an appointment, renew your prescription, or ask routine questions, please call during office hours. When calling our office to make an appointment, our receptionist will ask a few questions to help us prepare for your visit. We ask that you have your insurance information available when you call. You will also be told if a referral from your primary care physician is required. If you require a referral, your must bring it with you to your appointment. If you are calling to renew your prescription, you should call 24 hours in advance of running out of your medication. If you are calling to ask a question of your physician, our receptionists will direct your call to the medical assistant who will have an answer by the end of the business day. If necessary our physicians will call you back after business hours.
 
YOUR FIRST VISIT TO OUR OFFICE
On your first visit to our office, please bring our insurance cards, list of medications you are taking, and your referral form (if required).
Patients 18 years or younger are required to be accompanied by a parent or legal guardian.
 
CANCELLATIONS
If you must cancel an appointment, please call our office at least 24 hours in advance. This will allow us to give your time to another patient in need of our services.
 
OFFICE AND SURGICAL FEES
Our office fees are based on the complexity of your medical problem. We participate in most major insurance plans and you will be told if we participate in your particular health plan. We also accept cash, personal checks, VISA and MasterCard. You must pay any amount not covered by your insurance at the time of your visit.
If you have any questions regarding our fees, you may discuss them with our Clarkston business office.
If you require surgery and you have insurance coverage, we will bill your insurance company. Each insurance plan has different reimbursement conditions, such as deductibles and co-pays, for which you will be responsible.
If you require surgery but have no insurance coverage, please contact our Clarkston business office to develop a payment plan.
